Records Requests
Legal & Records Information (For Attorneys)
- All records, images, and reports are maintained by the treating clinic.
- The treating clinic contracts and pays us directly. We do not bill patients or insurance.
- X-rays are taken and stored at the treating clinic—we do not produce or store images onsite.
- We do not acknowledge an individual as a “patient” without proper signed authorization. Informal inquiries will not be answered.
